Care

Each resident has an individualised care plan which will state how the residents’ needs, in respect of their Health and well-being, are to be met.

The plan will be formulated in partnership with the resident and/or a nominated person and will be reviewed on a monthly basis by senior staff. All care plans are confidential and kept electronically and amended as necessary to guarantee any new needs/changes are met, to ensure changing needs are met and residents have positive outcomes.

Residents can have access to their own care plans any other person will require consent from the individual or from the Care Manager.

To ensure the focus remains on the care of the individual, identified Senior staff act as Key workers and have responsibility to take a special interest in their needs and be accountable for the continual delivery of assessed care.

Key workers will assist in care planning and will support the resident in meeting all their needs. Such as maintaining relationships with family and friends, as well as spend quality time with the resident both in and out of the home environment.

Faith and Culture

In regards to spiritual and cultural needs, residents will be offered all possible assistance to maintain and continue with the practises that were their source of strength and support, such as friends and their religion. Where possible, a resident may be able to attend the local church of their choice or for a minister/church member from that church to visit the home. For our residents from minority faiths, particular care will be taken to ensure access of cultural and religious needs as well as appropriate help to develop links with the local communities.
